Having a new baby? Call our office to schedule a prenatal visit to meet one of our doctors. The time is 5:30 until all questions have been answered. Please come visit us. We look forward to meeting you. Our next dates are: May 5 & 20, June 10 & 24, July 1, 2009.
When switching from another practice to White Oak Pediatrics you will need to have your child's records transferred to us. A medical release form is available to download. Having the medical records before your child's first visit would be great, however this is not required.
White Oak Pediatrics is now offering the 3 shot series of HPV vaccine. HPV helps protect young women from some types of Cervical Cancer. Please call to schedule an appointment.
Established patients: If you have any insurance, address or employment changes please download a new Registration form.The completed form can be faxed to 919-571-9314. Thank you.
New Patients: Please download both the Registration Form and Health History Form.This will save time on your first visit if the forms are completed upon arrival.
