A medical release form is available to download. Having the medical records before your child's first visit would be great, however this is not required. White Oak Pediatrics is now offering the 3 shot series of HPV vaccine. HPV helps protect young women from some types of Cervical Cancer. Please call to schedule an appointment. Established patients: If you have any insurance, address or employment changes please download a new Registration form.The completed form can be faxed to 919-571-9314. Thank you.
